To be approved as an agent for Wellington Risk Insurance Agency, LLC., all of the following are required:
- Agency Principal must possess a general lines or personal lines P&C license from the Texas Department of Insurance.
- Agency must possess an Internal Revenue Service Form W-9 with a valid Social Security Number (if agency is individually licensed), or Federal Identification Number (if agency is incorporated).
- Agency must have professional liability insurance coverage with liability limits no less than $1,000,000 each claim, $1,000,000 annual aggregate.

Wellington reserves the right to accept or deny an application in its sole discretion. Upon receipt and acceptance of the required information, a Producer Agreement must be executed. Thereafter, a User ID and Password will be issued for access to the Agent Portal.
